If you’re a faculty or staff member who can support the program by mentoring one or more students, please let us know with the form linked above. We will add your name and detail (and image if provided) and get back to you ASAP.

Mentor-Mentee Agreement Form

Once you’re matched to a mentor, take the initiative to reach out and set up a meeting where you can fill out this form, ideally as you talk to you mentor.

Travel Form

If you’re planning to travel for attending conferences or work, you can fill out this form. If your application is selected, you will get the adequate reimbursement from the organization.
